Agency: This included the Agency ID and Name.
Organization Name: This is the full name of the organization.
Affiliation Date: This is the date the organization joined LAGERS.
Fiscal MM: This is the fiscal month of the organization.
LAGERS Statement Contact: This is the name of the contact person regarding statements.
Hours Coverage: This is the full time equivalent hours elected by the organization.
Prior Service Coverage: This is the amount of prior service coverage elected by the organization.
Reporting Method: This is the method the organization elected by which to submit monthly reports.
Primary Address: This is the address where the organization wishes to receive mail correspondence.
Department ID: This is an identification number used to identify each department during employer reporting.
Description: This is the name of the department.
Actuarial Department: This is the type of the actuarial department used for reporting purposes.
Effective Date: This is the date the department was effectively recognized by LAGERS at the organization.
Benefit Program: This is department-specific; it includes the most recent benefit program elected by the organization which is different from the benefit program of the entire agency. If no difference between the department and agency benefit program exists, this field is left blank.
Name: This is the full name of the contact person and also acts as a hyperlink to open the contact person record.
Email Address: This is the full email address of the contact person; it also acts as a hyperlink to open the email application, allowing you to send an email directly from this screen to the email account provided.
Role: This lists one or more roles associated with the contact person.
Phone No.: This is the phone number on file for the contact person. This should be a direct phone number.
Phone Ext: This is the extension, if applicable, to be used in relation to the phone number.
Fax No.: This is the fax number on file for the contact person.
Address Line 1: This is the first line in your organization's address. This also serves as a link that, when clicked, will navigate you to the Address Maintenance screen (where changes can be made to your address information).
Address Line 2: This is the second line in your organization's address.
City: This is the city in which your organization is located.
State: This is the state in which your organization is located.
Zip+4 Code: This is the extended zip code used for mailing purposes. The +4 is not displayed if the information is not available.
Address Status: This is the status of the address. An active status indicates this address is valid and receives mail for your organization. And inactive status indicates this address is not valid and does not receive mail for your organization.
Account No.: This is the account number of the bank account you provided to LAGERS. This also serves as a link that, when clicked, will navigate you to the Bank Maintenance screen (where changes can be made to your bank information).
Bank Name: This is the name of the financial institution where your bank account is held.
Account Type: This is the type of account you hold (e.g., savings or checking).
Routing No.: This is a 9-digit number assigned to each bank location. This number is unique to your bank branch. Normally, you can get this number from your bank or look it up online.
Phone No.: This is the phone number of your banking institution.
Act Dept ID - Name: This is a display of the actuarial department and name in ID - Name fashion.
Effective Date: This is the date when the total contribution is effective.
Prior Service Rate: This is the actuarial rate applied for the prior service election.
Current Rate: This is the actuarial rate your organization has to contribute to cover current service employees.
Casualty Rate: This is the actuarial rate your organization has to contribute for disability coverage.
Total Employer Rate: This is the sum total of all components of the employer rate(s).
Employee Rate: This is the rate the employee has to contribute.
Benefit Program: This is the Benefit Program elected by your organization.
Effective Date: This is the date when the benefit program came into effect.
End Date: This is a system-generated end date for the benefit program.
Is Contributory: This flag indicates whether or not the contributory system was opted.
Effective Date: This is the date the election was effective from.
End Date: This is a system-generated end date for the contributory record.
NCR Elected: This flag indicates that the NCR was elected.
NCR Election Date: This is the date when the NCR was elected.
FAS Period: This is the Final Average Salary period; this will have an effect on how retirement benefits are calculated for your retiring employees. This can be either 3 years or 5 years.
Effective Date: This is the date when the FAS period came into effect.
End Date: This is a system-generated end date for the FAS period.
Rule-80 Opted: This flag indicates whether or not the Rule of 80 was opted.
Effective Date: This is the date the Rule of 80 became effective for your organization.
End Date: This is a system-generated end date for the Rule of 80 record.
Annual Valuation:
Year: The year the report was prepared.
File Name: The file provided by the actuary. The name format is YYYY_Agency ID.pdf.
Supplemental Valuation:
Year: The year the report was prepared.
Valuation Date: The year, month, and day when the report was prepared.
File Name: The file provided by the actuary. The name format is YYYY_MM_DD_Agency ID.pdf.
Initial Valuation:
Year: The year the report was prepared.
